Dementia and Alzheimer’s not only have a big impact on your loved one, but the rest of family as well. Our Nurses and Aides are trained to provide professional and companionate care to your loved one. We take the time to learn about your loved one and develop an individualized care plan that matches him or her with the perfect caregiver.
The Continuum Memory of care includes:
- Skilled Nursing Care – nurses (RNs and LPNs) are available to provide skilled nursing care, such as administering medication, ventilator management, wound-care, and other medical and non-medical services.
- Companion Care – Aides will provide light housekeeping, dish washing, laundry, dusting, pet care, and phone answering services for your loved one.
- Personal Care – many dementia and Alzheimer’s patients require personal care services, including bathing, dressing, grooming, toileting, oral hygiene, and more.
- Transportation – loved one may need to get to doctor’s appointments, do grocery shopping, or run errands, our caregivers will be there to help.
